Job summary
Sandringham Ambulatory Care Centre manages the Pathways program which provides assessment and support to community members who contract COVID-19.Nurse connectors provide direct telehealth support to COVID-19 patients. This role is suitable for a Registered or Enrolled nurse with skills in telehealth, care coordination and clinical support.
The role require a great understanding of the important role of supporting COVID positive patients in the community using telehealth.
Community pathways is delivered in partnership Alfred Health and Department of Health.
Working up to full-time including some weekends on a rotating roster. Minimum of 3 days per week. The role is primarily office based with some working from home occassionally.

These positions will commence straight away on a fixed term or casual contract and only shortlisted candidates will be contacted.
These roles work under Nurses Award 2020.
