Job summary
An exciting opportunity to join the Mid North Coast Regional Assessment Service with Aspire4Life as a Home Support Assessor. This role will see you support older Australians to continue living independently, by considering needs such as assistive technology, social support services, and community care. This is a community-based role allowing you the opportunity to travel throughout the Aged Care Planning Region each day to work collaboratively with clients in their homes under the Commonwealth Home Support Program(My Aged Care).This position will commence on a fixed term contract until 30 June 2024 (with the possibility for renewal based on funding). Candidates will need to be centrally located within the Coffs Harbour or surrounding area.
This is a full-time opportunity working Monday to Friday (no weekend or shift work).
We offer a comprehensive 2 weeks of full-time training and orientation held both virtually and face-to-face in our Coolangatta office (all expenses covered).

A typical week as an assessor with Aspire4Life: Utilise your assessment experience to conduct holistic style assessments using a client centered approach with a focus on wellness and reablement Capture vital client information and develop an individualised support plan underpinned by the clients unique goals within the clients home Develop strategies to connect people with appropriate services and supports in their community to further empower the client with their independence Working remotely within a region where you will be required to travel long distances between clients
About you: Experience in assessment and goal orientated support planning in aged, community, health or social services, and previous experience working in assessment and/or coordination of services Confident communication and interpersonal skills to engage and motivate clients, families, staff, and other stakeholders with a positive approach Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region Advanced IT skills with previous experience working across multiple platforms(Microsoft Office Suite) Previous experience performing in a KPI environment to ensure good quality client outcomes
What to bring? Demonstrated experience working in the aged or community care sector in an assessment-based role Tertiary qualified Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist with current AHPRA Registration or a minimum of a relevant Cert IV in Aged Care/Community Services. Unrestricted Australian driver's licence and be comfortable travelling long distances by car Ability to manage your time effectively in an autonomous working environment
