The Position
We are currently seeking an experienced NDIS Service Coordinator to join our team as team leader. The service coordinator will work closely with NDIS participants with disability, psychosocial disability, families/carers and support workers to deliver quality, person centered, and goal focused outcomes in an efficient and safe working environment.
The service coordinator team leader will ensure the individual needs of our participants are being met through the coordination of services whilst also implementing a planned approach to the professional development of team members.
Roles and ResponsibilitiesMonitor the coordination and development of individual support plan and ensure participants have a key support worker as per My Way policy.Develop individual roster of care, schedule rosters and approve timesheets.Allocate staff resources effectively, in relation to teams, shift swaps and leave planning using rostering software.Respond to Service Coordinators and Support Workers regarding roster enquiries via email, phone call, and text in a timely mannerBuild the capacity and resilience of the participant to support them achieve greater independence to self-direct services and supports in the longer termProvide the NDIA with reports on outcomes and success indicators within the agreed reporting frequency - Core and community services.Understand the individual support plan of the participant and design support plans while providing direct assistance when required.Coach, refine, reflect feedback and supervision to service coordinators and ensure support workers are competent and confident to support participants with complex health needs.Resolving points of crisis after-hours and on-call supports, planning, prevention, mitigation and actionDevelop and maintain positive relationships with all stakeholders, including families, and support workersPlan and facilitate regular and effective team meetings and professional development opportunities while maintaining a safe and healthy workplace for the team and the participants.
The successful Candidate will have:Relevant tertiary qualifications in the field of Disability/Social Work/Psychology Minimum one year in a similar roleDemonstrated strong experience and knowledge of NDIS, and working with people with disability, families, and carers.Demonstrated coordination, leadership and peer support skills.Ability to think creatively and use initiative, to work independently and as part of a multidisciplinary team.Strong written and computer skills for the creation of plans, programs, reports, correspondence, and other documents.Established problem solving and conflict management skills.High level communication, influencing, liaison and negotiation skills
