Job summary
An opportunity exists for enthusiastic and experienced Occupational Therapists to join our friendly, dynamic Community Rehabilitation Team (CBR). CBR has expanded to provide an Early Supported Discharge Rehabilitation Service (ESD). ESD is bed substitution program that provides intensive rehabilitation to patients discharged early from hospital.CBR ESD Rehabilitation program offers clients an innovative, blended model of care delivered via centre-based, home-based and telehealth sessions. Clinicians work collaboratively with the broader healthcare team to deliver holistic and coordinated multidisciplinary care to the people of the Western Suburbs.
The successful candidates will be well supported through supervision and education to develop clinical leadership skills as they supervise rotating Grade 1 Occupational Therapists and Allied Health Assistants, manage service portfolios and become a facilitator in our clinical education program.
1.0FTE FULL TIME PERMANENT
To be Successful in this Role you Have experience in providing goal-oriented physical rehabilitation to clients with a range of conditions, including neurological, musculoskeletal, chronic pain and aged related conditions. Demonstrate commitment to provision of occupation-based client centred practice Have experience supervising grade 1 staff and/or Allied Health Assistants or OT students. Have experience working collaboratively in multidisciplinary teams. Have experience in undertaking quality improvement activities Experience or ability to quickly develop skills in delivering patient care via telehealth (Health Direct platform)
