Job summary

To provide specialist nursing care and support for participants with high care needs that assists and facilitates them to live more independently. To ensure the health and clinical needs of each participant with disabilities are met in a way that reflects their individual needs and are delivered in a respectful and dignified manner.

Main Duties Providing care and support to clients in line with their personalised care plans. Assisting clients with their medication requirements. Working as part of the care team, using a holistic approach, to look at every aspect of our clients care. Provide high level care assessments and advise of implications for the care team. Provide clinical knowledge and expertise to support individuals accessing our Disability services. Develop and monitor health care plans around meeting the changing health needs of our clients. Proactively supporting clients independence and social connections while promoting positive health & well being.

Qualifications Current Registered Enrolled Nurse with AHPRA and evidence of continued professional development in accordance with the AHPRA standards. Current Drivers License. Current Criminal Record Check and Working with Children Check. Current CPR / First Aid Certificate. Qualification in Disability work or Welfare (desirable). Qualifications in Respiratory nursing or Critical Care (desirable).

Experience Experience in development and review of health support plans or other care plans. Experience supporting People with Disability who have complex and high support medical needs. Demonstrated Continuing Professional Development. Knowledge of continuous quality improvement principles

Skills Excellent verbal and written communication skills. Ability to build rapport and trust with clients. Strong time management and ability to prioritise. Ability to use Microsoft Office suite of programs and other computer systems for record keeping. Excellent problem solving and conflict management skills. Ability to work as part of a team and maintain good working relationships.
