As part of our exponential growth, we are seeking experienced Support Coordinators who are looking for a new opportunities.

Your primary purpose as a Support Coordinator is to support people who experience diagnosed psychosocial and Physical disabilities with complex trauma histories. By nature, this role presents unique challenges, and equally, deeply enriching experiences. This opportunity is ideal for an experienced and confident qualified candidate with the ability to proactively guide their own networks to enhance their portfolio in their region.

You will have the opportunity to work from home while working collectively with our team to ensure safe and ongoing supports to Participant's are met with overall integrity.

If that combination rings true to you, heres what well need to see:Demonstrated solid experience in a case management / Support Coordination role, working with people with a chronic and persistent mental illness.Have a minimum Certificate IV in mental health or its equivalent.Demonstrate the ability to independently plan, manage and organize own workload in a complex environment.Be a good communicator (both verbal and written) with the capacity to prepare clear and accurate reports and correspondence appropriate to the purpose and audience.Have high level problem-solving skills with the ability to work efficiently under pressure.Have well developed interpersonal and negotiation skills with the capacity to liaise effectively with a broad range of people and organisations.Have the ability to build ongoing referral pathways.

Technical assets:AHPRA registration + complete COVID-19 vaccinationCertificate IV in Community Services / Mental Health (minimum);Working With Children Check; Police Check, First Aid Certificate, NDIS Worker Check, NDIS Worker Orientation Certificate + complete, and;Current, unrestricted drivers license + own transport with a willingness to use your own vehicle if required.

Furthermore, this role involves regular liaison with affiliated services, key stakeholders, and the mental health first aid team. So a highly personable, accountable, and optimistic problem solver will succeed in this role.
