The Open Arms Outreach Program is designed to increase service accessibility for Open Arms clients. It establishes a network of accredited mental health clinicians, engaged through statutory registration, to support the delivery of services on behalf of Open Arms across Australia. ABOUT THE ROLE:
As an Outreach Program Coordinator, you will work collaboratively with clients and internal and external stakeholders to enhance Open Arms ability to identify and respond to the needs of vulnerable and at risk veterans, current and former Defence Force members and/or their families. Duties include:
- Intake assessment and referral, complex case management and case coordination to clients with identified complex and/or multiple needs
- Facilitation of the delivery of high quality clinical outreach services by ensuring appropriate referrals, applying quality assurance procedures and providing information and support to service providers
- Participate in community development and service promotion activities
- Maintain clinical records, collate and analyse data and prepare reports to assist in evaluating and improving service delivery
- Liaise with other government agencies and community organisations
ABOUT YOU:
- Qualified Psychologists, Occupational Therapists and Nurses must have current AHPRA registration
- Social Workers eligible for AASW membership and be eligible to apply for Mental Heath accreditation
- Minimum 3 years relevant experience
- Australian citizen (federal government requirement)
YOUR BENEFITS:
- $60 to $65 p/h +superannuation, paid weekly
- Monday to Friday 9am to 5pm (37.5 hours / week)
